When to Reach Out

Milk supply concerns often show up as:

  • Baby is fussy at the breast or feeds “all the time”
  • You’re pumping less than expected
  • Painful latch or shallow latch
  • Slow weight gain or frequent weight checks
  • Clicking, popping on/off, leaking milk, choking/coughing
  • Oversupply symptoms: forceful letdown, frequent plugged ducts, baby sputtering
  • You’re supplementing and want to rebuild supply


Important: Pump output is not a perfect measure of supply. We’ll look at the full picture: baby, breast, latch, and feeding pattern.

Milk Transfer Problems

If baby can’t efficiently remove milk, your body may downshift production. We assess:

  • Latch depth + positioning
  • Infant oral function (tongue mobility, suck coordination)
  • Feeding mechanics and swallowing
  • Nipple pain and trauma (often a sign of poor transfer)

Feeding Frequency + Timing

Milk production responds to effective removal:

  • Long stretches without milk removal
  • Scheduled feeds that don’t match baby’s cues
  • Early return to work without a pumping plan

Pump Fit + Pump Strategy

  • Flange size (huge factor)
  • Settings, cycle/suction, and timing
  • Hands-on pumping techniques
  • Workday pumping schedule that protects supply

Maternal Health Factors

Sometimes supply is affected by:

  • Postpartum hemorrhage, anemia, thyroid dysfunction
  • PCOS, insulin resistance
  • Retained placental tissue (rare, but important)
  • Stress, sleep deprivation, and inadequate calories/hydration
  • Certain medications (we’ll review safely)

Practical Tips While You Wait

  • Offer the breast frequently, skin-to-skin if possible
  • Aim for 8–12 effective milk removals per 24 hours (feeding + pumping combined) if rebuilding supply is the goal
  • If baby is sleepy or transfer is poor, add pumping after feeds
  • If oversupply: don’t “pump to empty” repeatedly, we’ll guide a safer approach

Frequently Asked Questions

Not necessarily. Cluster feeding can be normal, especially during growth spurts. We’ll assess weight gain, diaper output, and milk transfer.
