Tongue Tie & Frenotomy

Tongue tie is a common term used to describe a restrictive frenulum that may affect feeding, comfort, or oral function. In infants, concerns about tongue tie often arise when breastfeeding feels painful, ineffective, or more difficult than expected despite support.

Not all feeding challenges are caused by tongue tie, and not all tongue ties require treatment. Frenotomy Evaluation and Care

Frenotomy is a procedure that may be considered when a tight or restrictive frenulum affects feeding, comfort, or oral function. In infants, concerns often arise in the context of breastfeeding difficulties, poor milk transfer, or persistent maternal pain despite skilled support.


Care through Barrio Babies begins with careful medical evaluation. Not all feeding challenges are caused by oral restrictions, and not all tongue or lip ties require intervention. Our approach focuses on understanding the full feeding picture before determining whether frenotomy is appropriate.

When Frenotomy May Be Considered

Families are often referred for frenotomy evaluation when there are ongoing concerns such as:


• Difficulty maintaining an effective latch

• Poor milk transfer or slow weight gain

• Persistent nipple pain or tissue damage despite lactation support

• Clicking, milk leakage, or fatigue during feeds

• Limited tongue mobility affecting feeding function


These findings are considered alongside the infant’s overall health, growth, feeding pattern, and response to prior support.

Our Evaluation Process

Evaluation through Barrio Babies is comprehensive and individualized. It includes:


• Review of feeding history and current concerns

• Observation of feeding when appropriate

• Assessment of oral anatomy and function

• Consideration of maternal factors that affect feeding

• Review of prior lactation interventions and response


This process helps distinguish when oral restriction is likely contributing to feeding difficulty versus when other factors are playing a larger role.

Frenotomy as One Part of Care

Frenotomy is not a first‑line treatment for all breastfeeding challenges. When recommended, it is considered as part of a broader care plan that may also include lactation support, positioning changes, and follow‑up evaluation.


Some infants benefit from frenotomy, while others improve without procedural intervention. Decisions are made collaboratively with families, based on medical findings, feeding goals, and the infant’s response over time.

If frenotomy is recommended, families can expect:


• Clear discussion of findings and reasoning
